What is Virtual Care Management?

Virtual care management (VCM) is making it easier for patients to stay connected to the care they need, no matter where they are.  It involves using technology to engage patients who live in remote areas and also may face physical, financial, or behavioral barriers to being seen in person. Some programs also include Remote Patient Monitoring (RPM), where devices track things like blood pressure or glucose levels and share that data directly with the care team.

All of these tools work together to create a continuous connection between patients and providers, rather than limiting care to occasional face-to-face appointments.

Why Virtual Care Management Matters

For patients who have difficulty traveling or live in rural areas, VCM removes many of the common obstacles that get in the way of staying healthy.  Care becomes more convenient and less disruptive to daily life, which often leads to better engagement and adherence to treatment plans.

At the same time, providers benefit from having more consistent visibility into their patients’ health. Instead of waiting for issues to escalate, they are able to intervene earlier, adjust care plans as needed, and maintain closer relationships with the people they serve.

Many organizations see fewer hospital admissions, reduced emergency department visits, and overall improvements in patient outcomes, all while lowering costs.
