What is a Chronic Condition?
Chronic diseases are long-term health conditions that can have a significant impact on a person's quality of life. In order for patients to qualify for Chronic Care Management, the Centers for Medicare and Medicaid Services (CMS) requires patients to have two or more chronic conditions.
In this video, Kathryn Anderton, Director of Clinical Services at ThoroughCare, explains what constitutes a condition as "chronic," and how care coordination can help patients develop the motivation to self-manage their conditions. Identifying risk factors, promoting patient education, and providing medical reconciliation all contribute to the success of patients who are enrolled in a Chronic Care Management program.
