Care Management: A Patient's Story

Personalized care coordination, patient engagement, and consistent follow-ups are essential for care management to result in positive health outcomes. In this video, you'll see how one care management patient, Betty, utilized tools offered to her through care management programs to improve her overall health.  Through enrolling in care management services provided by 2-G Consulting in partnership with ThoroughCare, Betty's journey with care management has allowed her to take an active role in her own health.

"I've been working with my care manager for two years," Betty says, "and I don't know how I ever did without her".

"I think it's one of the biggest assets that have come for getting old...is having the security that someone's there to help you," says Betty.

Before being enrolled in care management programs, Betty was dealing with a wide array of health issues including complications from 30 years of arthritis, chronic back pain, kidney issues, and problems with her blood pressure.  Betty enrolled in the Medicare care management programs Chronic Care Management and Remote Patient Monitoring to receive personalized support for managing her multiple chronic conditions.  Along with gaining tools to help Betty manage and alleviate these issues, she was also assigned a care manager who regularly checks in to confirm her health issues are being managed.  Whether it is ordering Betty's prescriptions, sending her health information to other providers, or encouraging Betty to keep up with her health maintenance, she has been an essential part of Betty's care management journey.

"You know, you sit there and worry about something you have," Betty says. "Is it something or anything to worry about?  So instead of worrying about it, you call her and she can worry about it.  There are so many things that come up you think you have.  She'll go over them and either reassure you or assure you you should check into it."

Along with having access to tools and resources she did not previously have, Betty's care manager also acts as a trusted source of knowledge to guide Betty towards better health outcomes.  This allows Betty to be proactive when navigating her health issues, and has resulted in her avoiding unnecessary hospitalizations and emergency room visits.  Between this education, encouragement, and regular check-ins from her care manager, Betty now feels more in control and comfortable when it comes to improving her health.

"It keeps me out of the emergency room.  You feel like you always have someone there beside you because I can always count on her," says Betty.  "I feel very close to my body now since I have her."
