2025 Advanced Primary Care Management HCPCS Codes and Reimbursement
For 2025, the Centers for Medicare & Medicaid Services have launched a new program to assist healthcare providers in transitioning to value-based care. This program is called Advanced Primary Care Management, or APCM.
Advanced Primary Care Management focuses on preventing health problems before they become serious. It is designed to help healthcare providers shift to a value-based care approach. While it shares some features with other care management programs, like Chronic Care Management, its main goal is to enhance prevention and early intervention.
