Transitional Care Management (TCM) is one of several care management programs offered by Medicare. As a provider, you can use it to manage your patient’s transition from acute care to a post-acute setting, such as their home. The primary goal of a transitional program is to prevent hospital readmission.

In this video, we discuss the current rules and requirements providers should be aware of when offering TCM to their patients. From learning about the program's benefits, to understanding the three different services that are required for the program, we'll share the basic knowledge necessary to run a successful TCM program.
