Advance Care Planning: How It Benefits Patients
In times of emergency, or at the end of life, you might be unable to communicate your medical treatment preferences. Although you may assume that your loved ones know what you would want, that is not always the case.
In fact, research indicates that people only guess correctly about two-thirds of the time in making end-of-life decisions for their loved ones. That's why it's essential to have a plan in place to ensure your medical treatment preferences are known and respected.
Advance Care Planning, or ACP, helps adults of any age and health status identify, document, and share their personal values, life goals, and preferences regarding their current and future medical care. When done proactively, and updated as medical conditions or preferences change, ACP empowers everyone – patient, family, proxy, and providers – to make sound decisions with less stress and greater confidence.
