2025 Transitional Care Management CPT Codes and Reimbursements

The Centers for Medicare and Medicaid Services (CMS) updated the CPT codes for Transitional Care Management (TCM) for 2025. For 2025, there were no major changes to the TCM program rules and regulations in CMS’s Final Rule.

Transitional Care Management is a Medicare program to help patients avoid being readmitted to the hospital after a hospital discharge. TCM can be an effective way to embrace value-based care, so it’s important to understand the program's rules and regulations, as well as the different series of TCM codes your practice can bill for.

TCM CPT Codes

Transitional Care Management has two CPT codes for billing: 99495 for moderate complexity TCM and 99496 for high complexity TCM.
