2025 Remote Therapeutic Monitoring CPT Codes

Remote Therapeutic Monitoring, or RTM, is a Medicare program that aims to improve patient care and help people adhere to their therapy plans.  It has several billing codes associated with it. 

The reimbursement rates for the RTM codes are based on national averages, which may vary in your area. For the exact current rates based on your location, visit the Physician Fee Schedule website.

There are two categories of RTM billing codes, one for device setup for specific conditions and the other for time spent providing care services. 

First is CPT Code 98975. This code is for the initial setup and patient education on equipment use. It is only billed once per episode of care. The current national average reimbursement is about $20

The next three codes, 98976, 98977, and 98978, are the monthly codes for the device supply and data transmission specific to respiratory data, musculoskeletal data, and cognitive behavioral data. Only one of these device codes may be billed per 30-day period. So, if you’re monitoring a patient's COPD, you would bill the respiratory code and you would not be able to bill the other two codes within the same 30-day period.

98976 is for the device supply and monitoring of respiratory data with at least 16 days of readings in a 30-day period. It has a national average reimbursement of about $43.

98977 is for the device supply and monitoring of musculoskeletal data with at least 16 days of readings in a 30-day period. It has a national average reimbursement of about $43.

And 98978 is for the device supply and monitoring of cognitive behavioral therapy data with at least 16 days of readings in a 30-day period. The reimbursement rate varies by location - check the Physician Fee Schedule for your location’s reimbursement rate.

Now, we’ll look at the time-based RTM codes.

First is CPT code 98980, which represents 20 minutes of RTM treatment management by a physician or qualified healthcare professional in a calendar month. This includes at least one interactive communication with the patient and caregiver. The current national average reimbursement is $50

And finally, the add-on code 98981 is for 20 additional minutes of RTM treatment. There is no limit to the number of times this code can be billed per month, so it can be billed at 40, 60, and 80 plus minutes. The current national average reimbursement is $39
