Lumina Care identified a gap of psychiatric care for residents at nursing home facilities and wanted to provide a new program, the Collaborative Care Model (CoCM) to service the behavioral health needs for their patients.
ThoroughCare built out a Collaborative Care Management platform that enhanced Lumina Care's behavioral health offerings. Care plans were designed to specifically help behavioral health patients. Custom Analytics reports were created to identify eligible patients and properly bill for the program.
Lumina Care now offers CoCM and BHI services to 400 facilities and over 22,000 patients.
Lumina Care uses ThoroughCare AI to transcribe calls and see next steps in care delivery.
By automatically analyzing patient call data, care managers can connect more directly with their patients and drive care forward for better outcomes.
ThoroughCare tracks and reports Lumina Care's clinical, operational and financial KPIs.
