nursing home residents

Lumina Care partners with nursing homes to provide residents with care coordination and telehealth services within the facility, as well as after they transition back to the community.

These residents often live with complex chronic and behavioral health conditions and rely on multiple specialists and caregivers. Yet, many nursing homes struggle with limited staff and resources, making it difficult to consistently deliver the level of care these patients need.

Lumina Care recognized these gaps and set out to improve residents’ quality of life through proactive, preventive care.

To do this, Lumina Care focused on three key objectives for care management delivery

Lumina Care selected ThoroughCare as its care management technology partner

Lumina Care needed a technology partner who could support their care management needs and scale with them. ThoroughCare helped implement and scale Chronic Care Management and Behavioral Health programs. ThoroughCare Analytics tracked key performance metrics to inform management and care decision-makers.

400 facilities in over 30 states

Collaborative Care Management
improves behavioral health

Lumina Care identified a gap of psychiatric care for residents at nursing home facilities and wanted to provide a new program, the Collaborative Care Model (CoCM) to service the behavioral health needs for their patients.

ThoroughCare built out a Collaborative Care Management platform that enhanced Lumina Care's behavioral health offerings. Care plans were designed to specifically help behavioral health patients. Custom Analytics reports were created to identify eligible patients and properly bill for the program.

Lumina Care now offers CoCM and BHI services to 400 facilities and over 22,000 patients.

care management

By utilizing ThoroughCare and providing remote support and services, Lumina Care is able to help nursing home staff manage ongoing needs and stay closely involved in each resident’s day-to-day care.
