The Centers for Medicare and Medicaid Services (CMS) updated the CPT codes for Principal Care Management (PCM). For 2025, there were no major changes to the PCM program rules and regulations in CMS’s Final Rule.
Principal Care Management is a program that is designed for patients with a single, high-risk chronic condition that requires ongoing care. It provides care coordination and management services to improve patient outcomes while reducing hospitalizations and complications.

PCM CPT Codes
Principal Care Management has four CPT codes for billing: 99424 and 99425 for providers and 99426 and 99427 for clinicians.
