The Centers for Medicare and Medicaid Services (CMS) updated the CPT codes for the Annual Wellness Visit (AWV). For 2025, there were no major changes to the AWV program rules and regulations in CMS’s Final Rule.

The Annual Wellness Visit has three CPT codes for billing: G0402 for the Initial Preventive Physical Examination (IPPE), G0438 for the Initial Annual Wellness Visit, and G0439 for the Subsequent Annual Wellness Visit (SAWV).
