Starting January 1, 2026, CMS will require selected hospitals to take part in the Transforming Episode Accountability Model (TEAM).
Using ThoroughCare, hospitals can navigate TEAM from day 1.
We combine care management software with experienced clinical advisors to help coordinate better care and reduce costs.
TEAM incentivizes hospitals to effectively manage out-patient care, helping drive better outcomes.
For hospitals that reduce readmissions, shorten recovery times, and decrease costs, Medicare will reward.
The model targets five major surgical procedures:
Lower extremity joint replacement
Surgical hip and femur fracture treatment
Spinal fusion
Coronary artery bypass graft (CABG)
Major bowel procedures
Participating hospitals are required to coordinate care for patients after discharge to ensure a smooth, supported recovery.
CMS will compare your results against target costs and quality benchmarks.
To succeed, you’ll need:
Seamlessly between inpatient, outpatient, specialists, and PCPs
Document and track each episode of care accurately
Engage patients through recovery with proactive outreach
Meet or exceed CMS quality measures for both cost and outcomes
We sell software, but we’re not another software vendor.
