The average CCM reimbursement is $42. With our smallest plan starting at $225 per month, your CCM program pays for itself after treating just 6 patients.

Our CCM package allows smaller providers to manage patients at an a la carte rate, only charging you when you can bill Medicare.

For practices treating fewer than 50 CCM patients, we offer a $5 per billable patient per month rate.

For larger practices, our progressive pricing tiers allow you to manage more patients at a cost effective rate.

Our first patient minimum kicks in at 50 patients, with progressive tiers ranging upwards of 1,000+. Within these tiers, our per billable patient per month costs range from $2.50 - $4.50 dependent on your anticipated monthly volume.

2) Wellness Package

ThoroughCare's Wellness Package makes it simple to perform Medicare's Annual Wellness Visit (AWV). Like our other packages, our pricing is flexible depending on the number of visits performed each month.

Advanced Care Planning is included free-of-charge when performed during an AWV.

The average AWV reimbursement rate ranges from $120 - $265, dependent on the type of AWV completed. With our smallest plan starting at $200, your AWV program will pay for itself after 2 completed AWVs.
